LMWH Prophylaxis

• Stop 12 hours preoperatively. 

• Avoid restarting for at least 4 hours postoperatively and only if no bleeding evident.

Therapeutic LMWH

• Stop 24 hours peroperatively. 

• Avoid restarting for at least 24 hours postoperatively and only if not bleeding.
